Major/Essential Functions

Review discharge lists of patients from the hospital and emergency room to ensure that they are scheduled for follow-up appointments in the clinic in a timely mannerInform clinic personnel when patients require annual wellness visitsEnsure all home health and DME paperwork is addressed, sent to the appropriate physician, and returned to the home health agencyCollaborate with patient quality and improvement incentive programs
